Background and Objectives: The health system of a country, as one of the vital systems, is responsible for the health of the society, and for policy making with the aim ofto improving improve people's the community health and . Iimproving the performance of the health system is one of the governance duties of this system. This study aims to find the factors affecting the policymaking process of the health system in Iran based on "the second phase of the revolution" statement using an interpretative structural modeling (ISM) approach.
Methods: This is a qualitative study. Participants were 11 experts in health who were selected purposefully. By conducting interviews with the experts, the components were extracted and the relationships between these components were determined by the ISM method.
Results: Nineteen main components were identified at three levels. The third-level components included the managerial factors, human resources, processes and methods. macro policies, supportive role, tenure role, stewardship role, exclusive services, economic factors, social factors, technological factors, and political factors. The second-level components included prevention priority, health resource management, cultural enrichment, health promotion, and modification of methods and processes. The first-level components included promotion of community health and the economic efficiency of the health system.
Conclusion: Attention to the identified components can improve the policymaking process of the health system in Iran, reduce the problems of the health system, and cause economic benefits and social welfare.
