Effects of the head-down tilt position on blood pressure stability of the elderly candidate for open radical prostatectomy surgery

Introduction: The stability of blood pressure and maintaining of cerebral perfusion pressure are of particular importance in elderly anesthesia. However, there is a scarcity of clinical trials on the advantages of the head-down tilt position on the patients. This study aimed to evaluate the effects of the head-down tilt position on blood pressure stability of the elderly candidate for open radical prostatectomy surgery.
Methods: This randomized controlled clinical trial was conducted in Shahid Beheshti Hospital of Hamadan, Iran, 2022. Sixty elderly, who were candidates for elective open radical prostatectomy under general anesthesia, were selected by available method and randomly allocated to the head-down tilt and supine position groups. Patients in the head-down tilt position group were placed in a 15° head-down tilt before surgery. Patients in the supine position group remained in a sleeping position on the back during surgery. The mean systolic and diastolic blood pressure were assessed after anesthesia induction, after positioning, and consecutively in 30-minute intervals during surgery.
Results: The mean age of the patients was 60.9±4.2 years in this study, and their mean body mass index (BMI) was 24.7±3.5 kg/m2. There was no significant difference considering the mean systolic and diastolic blood pressure between the two groups in either of the measurements (P>0.05). The results of the repeated measures analysis showed that the variation of the mean scores of systolic and diastolic blood pressure were statistically significant across phases of the study (P<0.001).
Conclusion: According to the results of present research, the head-down tilt position maintains the patient’s blood pressure stability under open radical prostatectomy.
